Community Based Mentoring
At Mighty Mentors | Disability Services in Perth, WA, our Community-Based Mentoring program is all about building meaningful connections and fostering independence through real-life experiences. We pair individuals with caring, reliable mentors who provide guidance, companionship, and encouragement while supporting them to engage confidently within their community.
Our mentors help participants develop important life and social skills, explore new interests, and access community activities, education, or employment opportunities. Whether it’s learning to use public transport, joining a local club, attending appointments, or simply enjoying an activity of interest together, our focus is on empowering each person to reach their goals and build a sense of belonging.
Through consistent, person-centred support, we help individuals strengthen their confidence, expand their social networks, and take meaningful steps toward a more independent and fulfilling life.


Who do we support?
We provide compassionate and reliable in-home mentoring support to NDIS participants across the Perth and Peel region.
Our dedicated team supports individuals of all ages living with a wide range of disabilities — including physical, intellectual, autism, acquired brain injury, and psychosocial disabilities.
You are eligible to receive our services if:
- You are a participant of the NDIS and have a current NDIS plan
- You have a core (01 or 04) or increased social and community participation budget (09)
- These budgets are either self or plan managed
- You live within our service areas
- You have a goal stated in your NDIS plan to develop skills in the community and build your social network
- This support is linked directly to your disability that is registered with the NDIS

Common questions about our Community Based Mentoring
Can your support workers and mentors support 2 participants at the same time?
Yes, we can support two clients at the same time, which is also known as 1:2 support. This is possible as long as both clients have low support needs and do not display behaviours of concern that would be considered high-risk, so the mentor can safely and effectively support them both.
What is the difference between activity based transport and provider travel?
Activity Based Transport (ABT) refers to the distance (kms) driven in a mentor’s personal vehicle when supporting a client out into the community. ABT can only be claimed when the client is actually in the car.
Provider travel, on the other hand, covers the time it takes for a mentor to travel between client shifts. It does not include travel from the mentor’s home to their first shift or from their last shift back home. The most we can claim for provider travel is 30 minutes per client, per day.
At Mighty Mentors, we do not claim provider travel non-labour costs — which means we don’t charge for the mileage driven between shifts.
What categories can you claim from for support work and mentoring services?
To access our support or mentoring services, you’ll need to have funding available in either your Core budget (categories 01 or 04) or your Increased Social and Community Participation budget (category 09).
Category 01 is for in-home support only, so we can’t claim activity-based transport under this. If you want support to access the community, we’ll need to use funding from either category 04 or 09.
Will I be charged an establishment fee?
We charge a one-time establishment fee if we’re providing at least 20 hours of support per month and claiming from Core funding (categories 01 or 04). This fee will only be charged after you’ve received support from us for 3 months.
When will I be charged a short notice cancellation?
If you need to cancel a shift, we ask for at least 7 days’ notice. If a shift is cancelled within 7 days, the full cost of the shift will still be charged. However, you’re welcome to reschedule the shift to another time within 7 days of the original booking.
How our onboarding works

1. Screening
Our Admin Officer will review the details you provided in the referral form and contact you to confirm everything and ensure all criteria are met.
2. Quote
Once the referral information has been verified, our admin officer will provide you and all relevant parties with a quote for your approval.
3. Service Co-ordinator
Once the quote is approved, you will be assigned a dedicated service coordinator who will be your main point of contact at Mighty Mentors.
4. Service Agreement
A service agreement will be emailed to you for digital signing. It can also be signed at the initial in-person introduction meeting with your service coordinator.
5. Introduction Meeting
Your service coordinator will visit you at your home to introduce themselves, and to complete your personalised support plan and risk assessment.
6. Mentor Matching
Based on the information you have shared with us, we’ll find a Mentor who fits your needs and preferences. Then, we’ll arrange a trial shift to see if they’re a good match. If you’re happy with them, we’ll schedule your regular shifts starting on your chosen date.
